What To Expect
Please arrive 15 minutes early to your first appointment to fill out your patient forms or you may fill them out on the patient portal before you arrive. Check in at the front desk. Our helpful staff will walk you through the process.
What To Bring
Insurance, valid ID, and any previous health records that you would like to discuss with the medical provider. If you require a doctor’s note, ask the medical assistant that escorts you to the exam room and they will prepare one for the doctor to sign.
Financing & Insurance
✔ Marketplace Insurance:
Open enrollment is November 1st through December 15th, with the new coverage becoming effective on January 1st of the following year. Contact your agent to find out if you qualify for SEP which will be effective the 1st of the following month after the change. ✔ Medicaid/CHIP Insurance:
Open enrollment is May 15th through June 15th, with the new coverage becoming effective on July 1st. Contact a Health Program Representative to find out if you qualify for a special enrollment. To explore this option, please call 1-866-608-9422.

✔ Medicare Insurance:
Open enrollment is October 15th through December 15th, with the new coverage becoming effective on January 1st of the following year. Contact your agent for your Medicare options. For guidance, please call or text Jake at 801-400-8702.
- Plans we take: Aetna, BlueCross BlueShield, Humana
✔ Employer Insurance:
Contact your employer for your plan open enrollment and renewal dates. Check for out-of-network coverage on your Select Health coverage plan. You may be able to continue receiving services through your out-of-network coverage.
